It's difficult to say what makes the sight of someone smoking sexy to many man (and women for that manner). Some say that smoking is rebellious and gives that bad girl look (especially these days with smoking bans and all).

Could be that Hollywood might have to answer to that in some way. Let's face it, we all grew up watching our heroes smoke onscreen. Think Marlene Dietrich for example. It's hard to imagine her without a cigarette dangling from her lips. Then there's James Dean. Humphrey Bogart and Bette Davis. The nineties saw a very confident and sexy Sharon Stone defiantly smoke a cigarette in a police station while being interrogated in Basic Instinct. The examples are too numerous to name here.

Smoking fetishists seem very particular on their fetish as well. Most just love the way the smokers handle the cigarette. Some love the way they smoke (blowing smoke rings, blowing smoke out of their nose, the French inhale). Many are particular , wanting a long thin cigarette, even cigars or pipes.
